Information on Wellness Credits: Use this chart to track and apply your annual wellness points.
Make your flex, dental, and medical plan elections for 2018.
Earning your 100 wellness points allows you to retain 2017 premium rates.
All eligible employees must complete online enrollment even if you are waiving coverage and electing the cash option.
An in-network Primary Care Physician (PCP) must be designated for each covered family member when enrolling.
Employees and covered spouses must review the wellness tab for wellness points. If information is missing, it is the employee’s responsibility to supply the documentation for wellness credits.
For out of area dependents, please list their individual address under their name when enrolling for the out of area plan.
