• In order to enroll your student at Chisholm Life Skills, you will first need to have your current Child Study Team meet and determine if Chisholm is an approrpiate placement for your student. They must be 18-years-old and have completed their required graduation work in order to attend Chisholm. If your student has turned 18-years-old and they have completed their required credits, then please follow the following steps:

    1. Please print out, read, sign and return the attached enrollment documents.  

    2. Go to the enrollment site and enroll your student on line.
