P5320 Health Appraisal
Regular health assessments of all students are encouraged. In accordance with K.S.A. 72-6267, the school will inform parents of the required school entry health assessment, but will not exclude children from school who have not complied with the requirement.
Administrative Implemental Procedures:
- Every student age 8 and under who have never previously enrolled in any school in Kansas is required to submit results of a health assessment at the time of school entry.
- Lawful custodians will be informed of K.S.A. 72-6267 and its requirements for a school entry health assessment.
- Forms will be made available to be used for the required assessment, although non-USD 259 forms may be used provided they include health history, physical exam results, and screenings as medically indicated by the provider.
- Results of required health assessments will be filed in student’s cumulative folder and transferred on to the next school to which the student may transfer.
- Required assessments must be conducted by (1) a physician, (2) a person working under direct supervision of a physician (co-signed by that physician), or (3) a nurse certified by the Kansas Department of Health and Environment.
- Assessment may be conducted up to 12 months prior to school entry or 90 days after school entry.
- Lawful custodians will be referred to their private physicians or other local agencies for provision of these services.
- Continued assessment of health status by observations, screening procedures, partial assessments, and other inspections (e.g., dental) will be done by Health Services staff.
