NEW! WPS Health Services is now offering the Second Dose of COVID-19 Booster for Certain Individuals
The vaccine clinics will be at the Alvin Morris Administrative Center, 903 S. Edgemoor. Park in the south lot between AMAC and Curtis. Enter through Door #8. (Please check this webpage for new clinic dates that are added as needed.)
Vaccine Clinic Dates:

COVID VACCINE CLINIC QUESTIONAIRE AND CONSENT - Vietnamese
We offer the following options for vaccines:
- Ages 5-11 years old – Pfizer
- 1st and 2nd doses – at least 21 days in between 1st and 2nd doses
- 3rd dose for individuals immunocompromised – at least 28 days after 2nd dose
- This age group does not qualify for Booster doses yet
- Ages 12 years and older – Pfizer
- 1st and 2nd doses – at least 21 days in between 1st and 2nd doses
- 3rd dose for individuals immunocompromised – at least 28 days after 2nd dose
- Booster Dose – 5 months after 2nd dose (*Ages 18+ Can mix and match = can receive Pfizer or Moderna Booster)
- Booster Doses for those age 50 years old and older and/ or immunocompromised – 4 months after last primary series dose – may have 2nd Booster 4 months after 1st Booster dose (*Can mix and match = can receive Pfizer or Moderna Booster)
- Ages 18 years and older – Moderna
- 1st and 2nd doses – at least 28 days in between 1st and 2nd doses
- 3rd dose for individuals immunocompromised – at least 28 days after 2nd dose
- Booster Dose – 5 months after 2nd dose (*Can mix and match = can receive Pfizer or Moderna Booster)
- Booster Doses for those age 50 years old and older and/ or immunocompromised – 4 months after last primary series dose – may have 2nd Booster 4 months after 1st Booster dose (*Can mix and match = can receive Pfizer or Moderna Booster)
