Early Childhood Screening (ASQ Online)

  • Thank you for participating in our child screening/monitoring program. The questionnaires found at the links below are screening tools that will provide a quick check of your child’s development. The information you supply will help reveal your child’s strengths, uncover any areas of concern, and determine if there are resources or services that may be useful for your child or family.

    Who needs to complete this?

    As a parent, guardian or caregiver of a student attending pre-kindergarten or kindergarten in 2022-23, we ask you to complete the questionnaire by clicking on the link(s) in the table below for the building your child attends in fall 2022. You will be directed to a welcome page for your building, and then the link to the surveys. We will need you to complete both the ASQ: 3 and ASQ: SE-2 surveys unless you have been directed otherwise. Please try the activities on the questionnaire with your child and record what you see and any concerns you’d like to share.

    Why are we required to do it? 

    There are several reasons WPS asks families to complete these screening or assessment tools. First, the Kansas State Department of Education (KSDE) requires the collection of Kindergarten Readiness as part of the process school districts go through to be accredited learning institutions. During the accreditation process, this provides readily available data to discuss how prepared students are coming to school. Second, KSDE uses the data to assist in funding determinations for Pre-K programs. Third, Wichita Public Schools will use this information to ensure we have programs in place to meet the needs of all students coming into our schools.

    I think I've already done this; do I need to complete it again?

    If you have completed these questionnaires as part of your pre-school programming or with your physician, we apologize for the inconvenience, but you’ll need to complete it again. The assessment is specific to the child based on their age in months, so results from previous completions may be different.

    What will these assessments tell us?

    ASQ: 3

    • Communication: Your child’s language skills, both what your child understands and what he or she can say
    • Gross motor: How your child uses his or her arms and legs and other large muscles for sitting, crawling, walking, running, and other activities
    • Fine motor: Your child’s hand and finger movement and coordination
    • Problem-solving: How your child plays with toys and solves problems
    • Personal-social: Your child’s self-help skills and interactions with others
