Wi-Fi Device Checkout - 2022-2023

TO REQUEST A WI-FI DEVICE FOR THE 2022-2023 SCHOOL YEAR

1.      Complete the form below.

2.      Complete the student agreement and parent agreement (instructions at bottom of page)

    • If you checked out a device last year, you do NOT have to complete the student and parent agreements again.
    • If you did NOT check out a device last year, you need to complete the student agreement and your parent needs to complete the parent agreement.

3.      Check your email for directions on when to pick up your device.

 

INSTRUCTIONS FOR COMPLETING WIFI AGREEMENT

Both parent/guardian and student must complete separate agreements.  When completion of both agreements is verified, wi-fi device will be issued to the student.


STUDENT AGREEMENT LINK

Instructions:

  • Choose language at top right corner
  • Complete #1, 2 & 3
  • Read and initial #4 through 11
  • Check “Send me an email receipt”
  • Click Submit
  • Check your email for a receipt from Microsoft Forms and forward it to Mrs. Wilborn @ awilborn1@usd259.net

 

PARENT AGREEMENT LINK

Instructions:

  • Choose language at top right corner
  • #1 – Enter parent/guardian name
  • Read and click Next on the next five screens
  • Complete #2 through #7
  • Ignore #8
  • Read and initial #9 through #16
  • Complete #17 and #18 and click Submit
