Memorandum of Understanding (MOU) on COVID-Related Leave

A Memoorandum of Understanding (MOU) has been established for the 2021-2022 school year to assist employees who are fully vaccinated should leave be needed for COVID reasons. The MOU states:

Upon the completion of the 2021-2022 school year calendar, fully vaccinated employees who have provided proof of vaccination with EBIM by November 1, 2021 will receive up to 10 days of temporary leave time restored should the employee utilize temporary leave for COVID-related reasons restored at the conclusion of the 21-22 school year or awarded at the time an employee would begin to enter deduct status.

Please provide proof of vaccination no later than 11/1/21 to protect yourself for any current or future COVID leave needs:

  1. Send a photo of your vaccination card (should clearly show your name, vaccine type, dates, and the numbers associated with that vaccine that are on all standard vaccination cards) to  The subject line should be in this exact format for proper receipt:  VACCINATION CARD-LAST NAME, FIRST NAME-EMPLOYEE ID.  (Example:  VACCINATION CARD – SMITH, JOE – 12345).     
  2. When you report your COVID situation to our hotline, if your situation requires the use of leave, a form will be sent to you for completion so we have these on file for future leave day reimbursements and notifications of deduct/no pay status.  If you have already had a situation reported to us this school year that required COVID related leave, please contact us at to receive the applicable form. 

Vaccination cards are subject to confirmation of authenticity.  If you have lost your vaccination card, your first step would be to contact the facility or provider where you received it. 

By reporting your COVID sitautions in a timely manner, this can help us negate any COVID related absences from the attendance matrix should employees later go into deduct/no pay status.
