What Are Transition Services?
Secondary Transition is the process of preparing students for adult life after they leave the school system. It is the bridge between school programs and the opportunities of adult life including higher education/training, employment, independent living, and community participation. Transition is a "results-oriented process that is focused on improving the academic and functional achievement of the student with a disability."
Kansas requires Individualized Education Programs (IEPs) for students age 14 and older begin the process of planning for the student to transition from high school to post-school education, work, and adult living. By the age of 16, the IEP is to include a coordinated set of transition services designed to move students with disabilities successfully from school to post-school settings. Transition services are a coordinated set of activities that promote movement from school to such post-school activities as post-secondary education, vocational training, employment, adult services, independent living and community participation. They must be based on the individual student's strengths, needs, preferences, and interests.
